Filling out this form does not mean you have secured a spot with Wholehearted Handiworks for hand embroidery. Spots are limited from month to month based on availability and size of projects requested, and if your project is chosen for a spot, Katelyn will reach out via email or instagram direct message (whichever you note as preferred) for more information and to review the design plan and process. This form is intended only to serve as a request of a hand embroidery project to be completed by Wholehearted Handiworks.
By submitting this form, it is understood that you agree to the policies, terms, and procedures which can be viewed here.
Please fill out one form for each individual item with the only exception to this being if the items are a newborn hat in combination with a matching gown or blanket. If all three items (hat, gown, and blanket) are requested, please fill out two forms.
A non-refundable deposit of $25 is due at the time of stitch reservation, and the deposit will be applied to the total cost of the project.
Any item to be hand embroidered is to be received (in hand) by Wholehearted Handiworks no later than 30 calendar days prior to the desired deadline date. Please plan accordingly!
